Did you report this accident to the police? Come to that did the driver report it he is legally required too within 24hrs if you were injured.

Someone rang an ambulance for me, a paramedic came, then the police, then an ambulance. I thought it best I get checked over, but I didn't go to hospital since I was OK apart from the bruises.
The bruises are now faded, although still sore in those places and I can feel small lumps in my muscles. I took a picture of the worst one on the back of my thigh.
I don't have my own insurance but I am considering it. I walked away from this accident, but I might not be as lucky next time.

British cycling membership is £32 for ride, well worth it, get halfords discounts and 3rd party cover too

CTC offer similar, but not sure on cost or benefits

See Accident Advice(title only) for details on what to do.
With regards the bike, two quotes are required normally. Unless the bike is a write off.
If you do phone them again it may be worth mentioning, to them, that you're awaiting the forms from the MIB, with regards the claim as the insurance company named are dragging their feet, and you feel that the driver may not actually be insured.
