It's usually very easy to see the difference (if you're connected up properly with HDMI or component cables and not using Scart/S-video/composite connections).
However, some content on BBCHD is pretty dire as 'HD' and barely distinguishable from the same programme broadcast in SD on BBC1/2/4, so it's not necessarily as crazy as it sounds to be unable to tell the difference. Also, last year the BBC reduced the quality of its satellite HD transmissions (despite denials), meaning the very high quality images seen prior to summer 2009 are no more.
However, if people really cannot tell the difference after viewing a variety of programmes and HD/SD channels (and indeed Blu-Ray discs), then an eye test is probably required..
