From that photo I think it is a Pinion gearbox.

It looks like he's reversed the carrier for the larger sprockets, presumably in the interests of a better chainline on the one sprocket he's using.
Interesting cassette on the spare back wheel too (assuming the other spare wheel is front).

The rear rack is "interesting" too, when visible.
It looks to be a beam rack, with an assortment of bodged on bits to support and stabilise it

It's certainly a very impressive amount of luggage, but what does he do with it all - I mean, 7 vacuum flasks?
