Never done that. when I need a drink I do it when I stop at the traffic lights or pull over for a minute or 2,
I think I would fall of the bike if I tried to drink and ride the bike at the same time

Twice failed reinserting bottle into cage at lights, they were camelbak's too.

Never.. of course. but I did an even sillier thing.
Replacing it on a fast (ish) descent while freewheeling, I was finding it hard to get into the cage so I shoved and wrenched.
I tugged at the rear brake cable and locked up my rear with only one hand on the bars. Only for about a fifth of a second, but it was intense.
I am much more careful these days.
