In addition to adjusting fit when one buy's a bike or component like a saddle, how often do you adjust your fit?

Do you check it annually and adjust for physical changes in, you, the rider?

Once I've got it comfortable, I leave it alone.

I'll only adjust if I fit something new, like a saddle, bars or pedals, or if it becomes not-comfortable.
 
Hardly ever. After years of constant tweaking that didn't fix the fundamental fit problems, I had a custom bike built in 2013. I have not changed a thing on it, except while on tour in France last year one knee was playing up due to muscle fatigue, so I lowered the saddle slightly to prevent over-extension. That was comfortable, so I left it -- until today. After months of working with a personal trainer and physio to address alignment issues, I felt it might be safe to try raising the saddle 1 cm, so I did. We'll see how we get on. Otherwise, thousands of miles with no need to change anything.

My motto is, if it ain't broke, don't fix it. Get the fit right, then leave it alone. Unless of course you get a problem, in which it's safe to say it's no longer "right" (even if only temporarily) and
then it does need looking into.

ETA: This "When buying something that (A)ffects fit" option is a bit misleading. Surely when you buy something that affects fit, you take care to fit it exactly as the previous item? In which case, no tweaking or change is involved. Or shouldn't be. Right?

Once I'm comfortable I'll only tweak the set up if I change something on the bike that affects the setup, or if I notice that something on the bike has changed, my old charge spoon saddle is getting worn now, it has been on two bikes, and I noticed pressure where it shouldn't be recently so I tweaked the saddle position.

The cube mtb was perfect from the crate ,just seat angle and brake position needed a tweek,and thats never changed..the commencal gets fiddled with more often but nothing much..
My road bike is more problematic..but its getting there
 
I've changed fit about twice in the last ten years based on advice of bike fitters before that I did a bit more tweaking to get my initial set up.

My saddle position has been static across all my bikes for some time, but I often change bars and stems etc, particularly on my TT set ups. At the start of a season, often find last years position, unbearable and make changes, then over the season change them back again. Also made a few changes following broken collar bones to make comfort better.

From original fit and set up in current bike I have not changed the stem, bars, hoods etc. However I have constantly titted about withe saddle, it's height, angle, layback etc. I have tried various saddles and can't seem to find the sweet spot. The adjustments would be mm's mind, not big.

I might take a while to get the fit on a new (different) bike to my satisfaction but after that never change it again unless I change some items on the bike itself.!!!!!!!!!!!!!!!!!!!!!!!
