...so a reflector that fits on the pedal, but is covered completely by the shoe when you clip in, would be legal...?

Pedal Reflectors
Four are required, coloured amber and marked BS6102/2 (or equivalent), positioned so that one is plainly visible to the front and another to the rear of each pedal.

no comment there as to whether it has to be visible while the cyclist is on the bike though...?

On my recumbents it would only be visible to the occasional mouse or the Police helicopter as the pedals are vertical!
