Get my chain off the small inner ring this morning.... 
Think it was my fault, I was on the middle ring and by mistake when going up a hill change up to the larger (3rd) ring rather than the smaller one... chain came right off..
Tried a few times after and it worked perfect so I don't thing it is an adjustment issue...
