You need a chain tensioner such as a Singleator for it to work proper.

he might have got that magic gear. or used a half link, or maybe he has an eccentric bottom bracket or rear hub

Some unanswered questions here, interesting little bike. But no front brake and only (I assume) a rear drag brake? Scarey!
