Is that the failed managers themselves having to learn lessons, or their tool for dealing with any perceived mistake is to put their team members through the lessons learned procedure?

Having just been trhough this process I can heartily concur, all it makes you (and your colleagues who do the same job) determined to not own up when they've made a mistake and not get caught if they don't, the exact opposite fo the intended effect. That and when you have a near-miss, taking said occurrence and beating the person involved over the head with it until they collapse on the floor in the foetal position so you can put the boot into them.
Not that I'm bitter or anything :smile:

That sounds like a crap manager. A good leader will help those who have fecked up not feck up again and be supportive.

We have a no blame culture in work and it works . People are open and honest when things go wrong. We have reduced incidents significantly and the near miss system certainly prevents accidents.


Anyway back on topic . TheClaud is going to rip somebodies knackers off !!
