I still use metal tyre levers, I have two of those spanners in my tool box, though they don't get used very often.

If a rim has a name written on the side those evil looking instruments will ruin it so you best get the Rapha version that you can only use once without damage to rims and nails.

And the skinny cousin of the dumbbell spanner was the pressed steel bike spanner. You know, those ones which have a curve and a tab for undoing BB lockrings. I broke mine last summer  I was undoing an old BB and I whacked it with a hammer and it is no more..

I've never damaged a rim with a metal lever. I have however broken quite a few crappy plastic levers..

I've got one of those flat things you mention but I've no idea how to use it as anytime I've wanted to use it, it doesn't fit where I want it to! No metal levers but I've occasionally used the dumbbell spanner but anytime I've used it, I've set it down and had to work my way through all the potential holes again!

Which have all got larger because the stupid things were made from low grade cheese.
