I forgot the extra water bottle for my DD ride. I was upset as i thought it had dropped out on the way to the starting point and i would never see it again but I had just forgotten to put it on the bike before leaving.

I have set off on several occasions without pump tubes etc, I usualy realise about 3 miles into the ride, I just carry on and hope I don't puncture, I have also set off with a flat battery in the Garmin on several occasions, the phone comes in handy as a back up.

I have a few bikes, but use the same tool bike bag and pump for them all, I've got into a ride and realised that the pump or rear bike bag is still on the other bike at home. I usually turn back........

I can open my bag on Christmas day and there is still sunscreen in there. I have forgotton my overboots in the past but plastic bags work well as a substitute.

I leave a spare set and a shirt in work now having done the same. Fortunately I realised at Euston and there was still a TMLewins on the concourse then, so was able to get a shirt before cycling across town.

on the commute, my security pass. doesn't sound so problematic, but as both bike store and changing facilities are accessible only with the pass, it would have been no end of inconvenience; as i had my laptop in my bag i rode home and worked from there. only realised the pass wasn't in my jersey pocket 25km into the ride in...

Tyre levers. Unpacked my saddle bag at work a few days ago to check everything over before a big ride. Did the said big ride today (thankfully puncture free), got changed in work afterwards, and the tyre levers were sitting staring at me from my desk.
Whoops moment right there.

not quite forgot something more the opposite, I have a tool bottle with the gas pump for punctures (among other stuff) but still dare not go out without my pump for fear of not using the compressed air correctly. means I have a little bag on with pump in it still.

What's a "gas pump"? Can't you use this for pumping up tyres by hand? If not, in what way is it a 'pump'.
I use a 750ml water bottle in my seat tube bottle cage with pump, inner, levers, latex gloves; with spare space for eg spare rear light (and a couple of jelly babies (bagged) to keep my blood sugar up as I replace a punctured tube).

Spare tube and pump, I paid the price with a 4-5mile walk home, the worst part was having to take my cycling shoes off and feeling the slugs squash between my toes.

Well this could be called a 'gas pump' though they call it what it is:
CO2 Inflater and Pump
And if the CO2 doesn't work or it needs a bit more, or it's a slow puncture and goes down again; pump.
