The rear brake cable has been replaced and the brake is now operational again. It was strange seeing the man at the LBS getting frustrated with trying to get the cable through the canti yoke, I might get a better designed one to make passing the cable through easier.
Also should the washer be under the nut on the outside of the yoke or between the main cable and the inside of the yoke?
When I bought the bike it was like the latter and the LBS thought this was a mistake so he has swapped it to the outside, it was certainly a tight squeeze before getting the cable through the gap and the hole in the allen bolt at the same time as getting it to clear the link wire.

I mis-understood your OP. I think you have it in the right place protecting the yoke from the bolt, allowing it to be tightened without causing damage but I guess thats true of being under the allen bolt head too
I don't think it should be inside as there no room for it and the cable and if I think if it could be squeezed in, it would damage the cable. I'm guessing (I do a lot of that
) it was originally in-between to act as a spacer and stop the yoke getting crushed in transit.
