I think the beer ones employ particularly chipper folk to do the sober bit and it's probably not that much worse than working in a bar at 1am in the morning (or a Rickshaw taxi for that matter). The one I saw in Amsterdam had two 'designated drivers' probably because it looked like it carried a dozen people and was an unwieldy mass of tubing.
I suppose at least the majority of the sick hits the tarmac and you can just hose down the remainder.
I'm sure if there were a lot of them H&S would get involved but they probably inhabit a huge grey area where 'bicycles don't need seat belts' I'm pretty sure there aren't that many traffic laws aimed at large mobile tables. I would be interested in what they are insured (and generally classed) as though. Again, I'd guess the best fit is whatever legislation the cycle taxis fall under, which might not be very much.
