Wobbly headset.. Help!

Wow. That is described perfectly to a layman like me. Thank you very much. Can I please ask, can the headset get loose over time (six months) through general cycling, hitting road bumps etc? Are there parts of a bicycle need tightening up again over time?

Rack mouting bolts can sometime back out by a mysterious phenomena called precession. It appears as if someone has undone your bolts. My solution was to fit anti rotation ( star) washers. Most other vulnerable threads have the thread direction reversed to avoid the problem.
