Who cares, what they going to do, run you over? It's not as likely an outcome as you'd think. Cycle with confidence, you have every right to be there, you're not an obstruction.

Cycling with confidence is my normal routine, as for getting run over, it's happened a couple of times in the 40 years I've been cycling, its not pleasant.

Try not to let fear get the better of you, from looking at that road I'd say middle of lane is the only option. Anything else would be asking for trouble. That's just my opinion.

I've rarely been scared whilst on the road, the day that starts happening on a regular basis is the day I hang up my cycle shoes.

Do not touch suspicious objects
Whilst it does not make me any less confident, I know that I'm small and vulnerable. Unless there's no-one around I always look, signal, manoeuvre. I think that working on such assumptions as "they must know I will move over to overtake" or "they saw me look behind me, they must know" seem a bit silly when all you have to do is stick your arm out to avoid confusion.

Maybe if I'm feeling particularly small I ought stick a leg out as well?

I prefer both hands on the bike unless I need to. I signal turning right, I try and avoid signalling left to prevent left-hooks. Always look though.
