How lond do SPD cleats last?

Apolgies for yet another of my "piece of string" questions but my right foot pulled out of the M520's a couple of times last Week.

I still have the pedal resistance set to minimum from when I first went clipless and had never tightened them as I never felt the need.
Anyway, I've wound the adjustment screws 180 degrees and had no problems since, do I just keep tightening these down down when my feet pull out or should I think about changing the cleats?

I've done very little walking in them so far (furiously clutches plank of wood and refuses to let go.....)

My various cleats and SPDs are all over 15 years old - say 15000 miles for the most used - and still work just fine.
