They refer in the text to "contributory factors". I think this is massively misleading, due to how those "contributory factors" are recorded on RTC reports of the sort I regularly fill in.
There's a section of the booklet that I presume these stats are taken from as they have the same name "contributory factors". There's a fixed list to choose from (no free text option), and you can grade each option you select as only a) very likely or b) possible.
I may attend a left hook of a car on a bike, with the cyclist saying "he overtook me and turned into me" and the driver saying "he undertook me, I didn't see him and I turned into him".
In that situation I may well record a 'very likely' contributory factor of the driver failing to look properly, but as an independent evidence gatherer may also record a 'possible' for the cyclist undertaking. My write up would include my opinion (for whatever it was worth) and of, even if the cyclist did undertake does not exist the driver.
My point is that with the way things are recorded you are highly likely to get 'contributory factors' recorded for cyclists that may well not have even happened. Even if the stats Boris was quoting from were 'accurate' - they almost certainly weren't.
