My suggestion fwiw would be to set the saddle dead level with a spirit level.

Then if that doesn't help, do the same but with a thin plank or similar under the front wheel, so you've got an exact small tilt forward.

Repeat with thicker until either it's proved to be a useless suggestion, or your comfort has improved.

Many years ago someone here recommended I get a Selle Anatomica as I was getting actual sadle sores on my perineum. It is the most comfortable saddle I've every used and completely cured the problem. I've used one ever since and have now got three of them - one for each of my bikes. Not a cheap option but they often have seasonal sales.

I'm sure that other cutout saddles may offer similar levels of comfort.
