The main reason being you don’t propel yourself as you do running, it’s mainly about moving your legs fast enough to keep up with the belt speed (unless you have a manual treadmill). That’s not to say they’re easy.

Yeah, looking back at the times when I used one I set a pace about 25% faster than I do on the road. It seemed more like a sequence of jumps than a run.

Possibly controversial ;) I think that is the source of endless internet arguments with opposing people trying to convince each other "it's just basic physics, you idiot". I've no idea what the truth of it is.

But anyway. Back in the 90s I was a regular runner and I was working away on business. I didn't know the area and the hotel had a treadmill so I replicated my regular evening runs on that. I picked up a nasty soleus muscle strain even though I was just replicating distance and pace that I was comfortable with

My conclusion then was that treadmill running was (for me at least) sufficiently mechanically different to outdoor running to need caution as it was possible to overdo it even without overdoing it, if you see what I mean.

Possibly the stability side of things? I really struggle with my balance on treadmills (and also lose concentration and suddenly find myself almost off the back of the treadmill).

Lefty tighty. Get it righty.

My theory was that on the treadmill each stride was exactly the same, whereas outside it's all variable terrain and the repetitiveness led to the strain.

But it was years ago

Sounds reasonable, I get more pains running pan flat marathons than I do on hilly ones which I put down to the same issue.
