Most optical illusions make some kind of sense when you think about them, but how on earth does this one work?

I can't work it all out ... but it isn't a black and white image there is some colour in there. Our eyes get used to something after a bit and then we compensate for it, getting the reverse. Not sure why there is the 3 dots in there.

Yes, or positive, you just aren't as aware of it and most of us don't sit and focus on anything like that for 30 seconds at a time..
The original is a full colour image, just like the image that you see when you blink. The original has been chosen because the colours are muted and skin / cream so the negative appears monochrome and blue-ish which we mistakenly interpret into black and white.
I think most are "positive" about that.(Not that there is anything wrong with the young woman ...
