I try to remember to leave both rings in the middle when I have had work done at a bike shop, they leave it stretched from the highest at the back to the lowest at the front.
Smaller front and smallest rear, same as your LBS, on the basis that leaving the springs in the dérailleurs under tension unnecessarily seems undesirable.
Smallest front and second smallest rear. The smallest rear isn't an option because if/when I roll the trikes backwards they climb out of the smallest rear position, but not the second smallest.
As long as it's not cross chaining, that's acceptable. Cross chaining is the work of Satan, ANY cross chaining will result in the spontaneous explosion of the bike. Ohhhhhh the humanity, sprockets and bits of chain, and bottom bracket parts alllllll over the place.
Whenever I park my bike, I always make sure it's in the big ring and middle of the cassette, so the derailleur cage is vertical (draw an imaginary line through the centre of the jockey wheels).
I also ensure both tyre valves are at 6 o'clock and the cranks parallel to the ground, right crank pointing forward.
It's important to get these things right in case any passer-by stops to look at my bike - can't have them thinking I'm an amateur.
