Accidental is one thing, careless is another, but there was one notorious ascent, which may or may not have been the matterhorn where the two groups were deliberately trying to cause their rivals harm.

I'm guessing they are all adults.
Their bike, their problems.
Help as and when you can but it's not your responsibility to look after every detail.

I’d say as ride leader you of course need to be capable of helping with puncture repairs and sorting basic mechanicals. Also checking bikes for road worthiness before setting off

I'd only expect that on a led ride with a paid leader.
