OP if your wife also has an iPhone you can use the "find friends" feature. It's free but it does rely on getting data coverage. One advantage (or disadvantage) is you can see each other's location at any time.
 
Find My Phone, or whatever its called. Set it up, give your Missus the passwords etc

Apple uses "Find my Friends"

You send an invite, that is accepted, so both parties consent. You can also stop sharing if you wish to sneak off for a quick pint

Problem is that as with most Apps of this nature it relies on a good phone signal and can be inaccurate if not updated or signal is poor

There is a whole market for tracking the Elderly and Dementia patients, this may be a route to look at. Some also have a "fall detection system" that would presumably alert in the case of a crash.

In SAR as a management tool (not an actual live tool) we use Viewranger and Buddy Beacons. Whatever, if you use a phone based solution get yourself a nice beefy power pack, because one thing smartphone don't like is actually being used and guzzle battery.

If your phone is flat in an emergency, knock on the nearest property and ask to use their phone.
At times round here you could be an hour* away from the nearest property. Unless you're willing to chance a road/track you don't know.

*At walking speed.

@PaulSB, can't comment on stents or their impact, but don't let epilepsy put you off cycling. I've had one specialist tell me I'm less likely to have a siezure doing something I like doing. I've never had one whilst cycling.

No different to days of old and we alll survived then without a phone on our person

The OP is wanting a reliable tracking device to reassure his wife following his epilepsy diagnosis. So in the event of his potential emergency, knocking on someone's door is not going to work.

In those situations that remote it'd also be likely no phone signal.

I've not used Endomondo for several years, however I have in the back of my mind that this supports live tracking and enables folk to track your rides "real time".

SPOT is probably most reliable is almost any location. However, as a 62yr old that regularly goes cycling/walking/climbing on my own I just take my phone. If I am going on an overnight trip, I don't give my exact route to my family, as I may need to change my mind at the last moment because of weather, etc. I just tell them "If I fall off, you get to keep all the bits".
