While I realise this whole ‘out of the saddle’ thing is a . . . thing, perhaps after having gone to the trouble of setting the bike up so you can perform efficiently in a sitting position, it would be best to use it like that? Especially if standing while riding causes problems.

It doesn't normally, the particular bike I had the issue with I set up mistakenly by using saddle tip to stem which works on everything else I have but this particular bike must have a really steep seat tube as when I checked saddle setback from the BB it needed to be 2 cm further back
The hill in question was a very steep short ramp so out of the saddle is the best way to tackle it.

Knee definitely improving , i managed to ride up and down the road yesterday and it felt a lot better .Im not going to push it though i might go for a little spin tomorrow .

Probably because I am getting old so the leg hair has all moved to my eyebrows, nose, and ears! :laugh:

I messed up my left(?) knee in '19 after buying my first set of 3-bolt shoes and setting the cleat up incorrectly. Went for a ride in the hills, slight niggle when giving it beans, got much much worse after the ride.

Ended up doing gentle rides on Zwift for a month ish instead after tweaking the cleat position (point more towards my big toes), then gradually increasing distance and hill beans outdoors.

I'm going Occam's razor here.

Arthritis of the knee cap. Stretch hamstrings and quads twice a day - massage supporting ligaments with ibuprofen gel.

Following an RTC I have this arthritis. I can vouch for the stretching but only found the gel, which is highly effective, necessary in the first few weeks after the crash.

After a week back commuting the knee is improving steadily and i did mange to ride out of the saddle friday but i did push it a bit harder than I should have so friday afternoon the niggly feeling was back when seated so i have decided to rest over the weekend rather than going out for extra miles even though i really wanted to ride.Still 95 miles for the week is a good start compared to when i couldnt even turn the cranks with no pressure .

Forget the doctor (GP) and go and see a Sports Physiotherapist

Went back to the doctors saturday at a different clinic and this paramedic actually looked at the knee and said its likely i have sprained a ligament so hes prescribed naxproxen , rest and a referral to a physio. Certainly feels better than last weekend when i did push the exercises too much and set myself back.

When I get a sore knee, which thankfully quite rare, I use icepacks 6 times a day and ibuprofen for anti inflammatory treatment. I then do very light pedalling until my symptoms completely clear
