Bottom bracket thread? I had that once. Removed the bottom bracket, put a couple of turns of Plumbers' Tape For Everything on the cups, re-fitted it. Silence.

But it's only in certain gears, not all the time. Although could certain gears but certain strain on the bb?

I had a problem like that first one was a broken water bottle holder, then it was a dry chain, that was more noticeable when pedaling hard and felt that more through the right foot, the best one was a saddle bag, the back of my thigh was touching it every revolution and it was the velcro rubbing on the seat post, i totally empathize with you, a proper head doer-in when it happens, you will find it eventually.

I've just tracked down a really unpleasant creak on my road bike. Smallest of the two bolts holding the rear mech hanger was loose and doing a fantastic job of transmitting an intermittent clunk and groan through the frame.
