The picture of your bike brought a tear to my eye!
Like others have said keep a daily journal noting all your aches and pains as it'll be hard enough to remember such details 6 months down the line. Your mind tends to forget nasty events. Photograph everything damaged; bike, helmet, clothing, panniers etc.
Keep all out of pocket expenses for meds etc.
When I had my off last Sep I had Cyclemeter running on my iPhone I was able to email the gpx file and load it into Memory Map. I could then determine my plot, speed and time of collision.
