Zwift defaults to 50% trainer difficulty on install. You can change in settings. Pros and cons for doing so.
Check weight and height as BL suggests.
Hope you recover quickly from your injuries 👍
Does that only apply to smart trainers though?

@cyberknight - the thing to remember is that your power meter is the source of truth for your performance. All that matters indoors is hours and powers. Speed is an estimate at best and not useful for comparing to outdoors. When comparing efforts indoors it is better to use something like intervals.icu to see time spent in zones rather than speed. Strava premium might do the same thing but I don’t subscribe so not sure.

What tyre pressure are you running? I found I had to have a very specific tire pressure (100psi) and tighter on the back wheel to get the recommended run down for th kickr snap

None of that should matter if @cyberknight is using a powermeter for his Zwift Input on a dumb trainer.

Mainly Zone 5 HR for 164W suggests either:
The powermeter is reading low
The HR monitor is reading high
You have low FTP / power (doesn't sound that way with your real world riding) or you weigh about 40kg

Zwift's agorithm is roughly right (albeit it assumes no stopping at junctions, no slowing down at corners and no wind) - so it can't really be that.

Bear in mind is just alters your gearing with respect to the gradient, basically at 50% it makes 10% gradient 5% so you don't need to jump down the cassette to find the correct cadence for you.

Finally if you're using a power meter 4ii, then with respect, you're probably not as fit as you thought you were. Given your injury its not unusual to be below par. I'd be doing what @berty bassett suggested, ride easy, get a good base of miles, then when healed, build power.
